- With which faculty member would you initially wish to work and why? If you have no preferences at this time, also let us know.
- Provide any other information that you believe would further support your application.

Human Factors Questions
- Please list life, work and research experiences relevant to your application, and describe the nature of those experiences.
- Please briefly describe your writing and quantitative skills. Example documentation should be submitted as supplementary material. This may include papers submitted for course work such as literature review papers, research methods and analyses papers or proposals, publications and/or R-code scripts from analysis projects where you played a primary role.
- What do you hope to achieve during your graduate education?
- What kind of work in psychology would you like to be doing seven or eight years from now? Detailed information concerning specific applied and research interests would be helpful.

Writing Sample
If applying to the Clinical or Human Factors program, please provide a writing sample, such as a published article, thesis, or class project.
Applicants to the Community program do not need to submit an additional writing sample.

You may also include any supplemental information about your honors, scholarships, employment, volunteer work, as well as any articles, presentations and/or projects if you feel this would strengthen your application.
